In-Person Visits

If you live in Manhattan or the Brooklyn Heights, Cobble Hill, Prospect Heights, and Park Slope sections of Brooklyn, New York City, then I will make a House Call Visit to you (and whomever else you designate.)  New patients must fill out the Nutrition Assessment Form before the appointment.  You can complete the form online and simply save it.  I will receive the form automatically.
